Home Care Program
Life may be challenging at times,
especially as you get older or have special needs.

In-home services directly support seniors and disabled adults by providing comprehensive assessments and personal care plans that allow clients to live independently at home. Our Home Care Coordinator talks with the person and/or family who needs help, and together, they decide what services are needed. Our ultimate goal is to keep our clients in their homes and as independent as possible while providing them and their families with the necessary services to age with dignity.

In-home services include:

Homemaking – Light housekeeping, laundry assistance, and other necessary tasks

Personal Care – Bathing and grooming assistance

Respite Care – Caregiver relief from daily caregiving responsibilities

Counseling - Specialized assistance in aging issues like depression, grief and loss, and caregiver support

Consumer Directed Services (CDS), although similar in nature to in-home services, these services are provided to persons with a physical disability and do not include any task performed by a licensed professional. A participant of CDS must be able to direct the care planning process and hire an attendant. In general, CDS consists of personal care (bathing, cleaning, and meal preparation).

Specialized In-Home Care Services - We provide specialized in-home care services for individuals with brain injuries (Brain Injury Waiver), families caring for an individual with dementia (Structured Family Waiver), as well as individuals living with HIV or AIDS (HIV/AIDS Waiver).
