Circle of Care, St. Louis is one of the leading agencies providing personal support services in the St. Louis metro area. We are now a Missouri Structured Family Caregiving (SFCW) provider. The Missouri Structured Family Caregiving Waiver provides assistance for physically or cognitively impaired adults with Alzheimer's Disease or related dementias who are at risk of institutionalization (nursing home care) but can live at home with long-term care services. We are the only agency in the St. Louis area providing health coaching beyond the standard program. Our program offers a health coach for day-to-day communication and coaching. Our team includes a nurse to provide clinical experience to meet the client's needs and a case manager to provide the client with the maximum level of support. 

 

Allowable Services and Supports

Supportive and health-related attendant and homemaker services that substitute for the absence, loss, diminution or impairment of a physical or cognitive function. Services related to needed Instrumental Activities of Daily Living (IADLs) and Activities of Daily

Living (ADLs);

  • Homemaker: general household tasks like cleaning and laundry

  • Attendant Care: assistance with ADLs like bathing, dressing, and other personal care tasks

  • Medication Oversight: managing medications and doses (to the extent permitted
    under State law); and

  • Escorting for necessary appointments, whenever possible, such as transporting
    individuals to doctor appointments and community activities that are therapeutic
    in nature or assists with maintaining natural supports

Eligibility Criteria
All SFCW participants must meet the following eligibility criteria:

  • Be at least 21 years of age and over;

  • Diagnosed with Alzheimer's or related disorders as defined by state statute 172.800 RSMo by a physician licensed in the State of Missouri;

  • Lives with the person providing their daily care

  • Need assistance with daily living activities such as feeding, dressing, bathing

  • Need five or more hours of care

  • Need help with ADLs or homemaking activities

  • Be Medicaid-eligible in Missouri
