Info & Referrals (Case Management)

Provides information on available resources, identifies client needs, assists with referral/application to government and community programs, and ensures needs are being met

Emotional Support & Counseling

Delivers supportive services, such as regular phone connection with trained volunteers, and specific counseling services for clients and caregivers

Non-Medical In-Home Care

Provides assistance with grooming, bathing, dressing, medication reminders, mobility, and transfers (such as from a wheelchair to a toilet, chair, or bed)

Scheduled Transportation

Offers comfort and peace of mind by delivering affordable, passenger-focused transportation services through reliable, volunteer drivers

Safety & Wellbeing

Delivers supplemental food and supplies for homebound, low-income, and vulnerable clients, as well as crisis intervention and problem-solving therapy to those with immediate needs and in complex situations

home and offer support to caregivers and families through a variety of programs.

Home Care Program

​

At Circle of Care St. Louis, our Home Care Program is designed to empower seniors and adults with disabilities to live independently and securely in their own homes. By offering a personalized approach to care, we provide a valuable alternative to full-time institutional living, enhancing the quality of life for our clients in the familiar surroundings of their own spaces.

 

Our Program Services Include:

​

  • Activities of Daily Living (ADL) Support: Our caregivers assist with crucial personal care tasks such as bathing, dressing, and grooming, essential for maintaining hygiene and comfort.

  • Instrumental Activities of Daily Living (IADL) Assistance: We help manage daily household chores, including meal preparation, grocery shopping, and light housekeeping, to ensure a well-kept living environment.

  • Mobility Assistance: We provide support for safely navigating the home, enhancing overall safety and helping to prevent falls.

  • Respite Care: Our respite services offer temporary relief to family caregivers, giving them the opportunity to rest and recharge, knowing their loved ones are in good hands.

  • Medication Management: Our dedicated personal care attendants provide timely reminders to clients, ensuring they take their prescribed medications as scheduled.

  • Nursing Visits: Regular visits from our nursing staff include services such as medication setup, diabetic nail care, and general health assessments to ensure continued well-being.

We work to eliminate social
isolation and loneliness.

Social isolation is the lack of connections or feeling
connected with other people. Loneliness can occur
even without social isolation. Factors such as living
alone, loss of family or friends, chronic illness, and
hearing loss put older adults at an increased risk for
social isolation and loneliness.

Recent studies have shown that social isolation and
loneliness put older adults at a higher risk for serious
medical conditions, including depression, dementia,
heart disease, and stroke.
