Identifying Changing Needs | We pay attention to changes that may affect your safety, independence, or ability to remain at home and help determine what follow-up may be needed.
Individualized Care Planning| We help identify priorities and coordinate appropriate next steps based on your needs, circumstances, and existing services.
Healthcare & Provider Coordination | When appropriate and authorized, we can communicate with healthcare providers and other organizations involved in your care to help address concerns and support continuity.
Home Care Coordination | We help connect what is happening during Home Care services with the larger picture of your needs, supports, and care.
Social Needs Coordination | When issues such as food, transportation, housing, utilities, benefits, or other social needs begin affecting your ability to remain stable at home, we help connect you with the appropriate support.

When a Home Care client develops needs that extend beyond the scheduled visit, our team can help identify what is happening and connect the client with the appropriate next level of support.

That may include Care Coordination, Benefits & Resource Navigation, Case Management, Transition Support Services, or another community resource.

You do not have to navigate every change by yourself.

Case Management provides ongoing support when multiple barriers are affecting your goals and you need a dedicated Case Manager working alongside you through an individualized Case Management Plan.

Transition Support Services provides focused support when you are moving through a significant transition.
