Sometimes You Need More Than Navigation
Benefits & Resource Navigation works best when you have a specific benefit, program, resource, or immediate need that you're trying to access. If your circumstances involve several interconnected challenges and you need someone working alongside you toward goals over time, Case Management may be more appropriate.
If you currently receive Circle of Care Home Care and need help connecting the different parts of your care and support system, Care Coordination may be the better fit.
You don't have to figure that out before contacting us. We'll help determine the right starting point.
