Circle of Care St. Louis provides an integrated continuum of care for older adults, adults with disabilities, transition-age youth with disabilities, and grandfamilies.
Through Medicaid Home & Community-Based Services, Professional Social Services, and Community Wellness, we address the physical, social, emotional, and systemic barriers that can threaten a person’s independence or a family’s stability.
